"Images saved at higher than 72 dpi open up in Preview as their "Actual Size" which means large scans show up as tiny tiny thumbnails."

That explains the mystery I'd been trying to figure out, thanks! Mail does this too, I get photos sent from one person, and they display fine inline in Eudora, but tiny in Preview. Then he reported someone using Mail viewed them too small. I couldn't figure out what was different about his jpegs until I read your comment. They are at 150 dpi.

Did you try checking the "ignore DPI for actual size" checkbox in Preview's preferences?
