On MacOS X 10.4.8 on an iBook G4, simply open Audio MIDI Setup.app ( found at /Applications/Utilities/Audio MIDI Setup.app) and click the "Thru" checkbox in the listbox on the lower left. I've heard others say that this feature isn't available for them, but it works for me on 10.4

There's also a great freeware app called "Audio In" ( listed on VersionTracker.com) which is mainly a simple audio recording program, but it also support Play Through and display a live waveform of the input device.

I find it quite handy for recording live shows, then I use a program like Garage Band or Audacity to split up the recording into tracks. Works great for making live CDs of local bands I've worked with.

Needed for new Macs

As osxpounder posted above in response to another nay-sayer, "On some systems, including my all-stock MBP, the check boxes to which you refer remain greyed out, unchangeable, and therefore LineIn is a useful solution for my machine."
My stock MacBook has the boxes greyed out as well. I've had LineIn installed since I got the new Mac, since even on a beast of a machine like this it can be a pain to open GarageBand. It's still pretty bloated.
