Easily create 128x128 PNGs from icons
You can usually get the hi-res 128x128 image of any application icon by invoking "Show Package Contents" and navigating to the contents/resources folder. This is where the .icns files are stored. They can then be opened in Preview where it is possible to choose the resolution image you want. Just make sure the sidedrawer in Preview is open so you can see all the images that the .icns file contains. You can then select the 128x128 image.

MacFox, that's a great additional tip for users like me, who have lots of Dock icons and don't want to clean out the Dock just to snag an icon. With so many icons in my dock, the OP's tip just gives me fuzzy icons.

Custom folder icons are stored in a hidden file within the folder named "Icon" with a capital I and trailing CR (^M). But much more useful than that factoid is Gerti's hint below, which is what I've been doing. And then guardian34's method (farther down) is even faster and easier! Just select in the Finder, copy, go to Preview, ⌘N, and save in your desired format.
