I've had menu items disappearing for about a year...I've gotten so I just re-check them in the associated App or Preference. I once found many comments and a few solutions on Apple's Support Boards. Try Searching under:
"Mac OS X>Mac OS X v10.4 Tiger>Finder, Dock, & Dashboard> Menu items missing".

Here are a couple from there, suggesting these presumably corrupted files be deleted:

/Users/<yourname>/Library/Preferences/com.apple.systemuiserver.plist

/Users/<yourname>/Library/Preferences/ByHost/com.apple.systemuiserver.xxx.plist (where xxx is a 12 digit hex string)

"The loginwindow.plist file in your Home > Library > Preferences folder..."

They suggest logging out and back in after making these deletions, and if that doesn't work, to do the same and then delete caches as well:

"In /Users/<yourname>/Library/Caches, delete everything.."

This will not *recover* your menu items, just wipe out everything. You will need to then re-add them.
