I think you misunderstand.

In IRC, where this convention originated, conversations typically flow, depending on the IRC client you're using, something like this:

alice: hey bob!
bob: hello, alice, what's up?
alice: i'm breaking into your computer!
bob runs away
alice laughs
alice: just kidding, sorry
bob: phew!

Etc. In other words, it's not just that "/me" is being "expanded" to "person does ...", but that the action itself is represented as a break in the dialogue, rather than just a different kind of statement.

This isn't really a macro that's being substituted; it's actually an IRC / Jabber / etc command.
