As Rob noted, Americans also have the "flat bin" mailbox. Mailbox_en is actually used in the toolbar (control+click Mail's toolbar, select "Customize toolbar...", and you'll see the Mailbox_en icon to the left side). I don't know what "en" stands for, but apparently not English. ;-)

No, it looks like _en does stand for English. If you set your primary language to Japanese, or French, or German, and then launch Mail, the mailbox toolbar icon will be the flat in-box.

You can change your primary language on the fly in the International preferences.

Right, and I think there is a obvious reason for the different icons. The 'English' mailbox_en.tiff icon shows a typical American mailbox that is not seen in other parts of the world. Living in the Netherlands I will stick to the more neutral mailbox.tiff icon.

I think he meant "English" as in "from England".

Anyway, my primary language is US English, but Mail displays the "flat bin" mailbox in the side drawer.
