I layout CD artwork for a living and our major label specs we use are as follows:

The "inlay" or "tray card" of a standard CD jewel case is a rectangle of paper 138mm wide by 118mm high.

The two thin spines add another 6.5 mm each to the width for a final dimension of 151mm wide x 118mm high.

Add trim marks around the perimeter of this rectangle, also try adding a couple of fold marks above and below where the two spines start.Before you trim out your artwork score the spines using an unserated butter knife and a ruler. This will make it easy to fold accurately prior to insertion into the tray.
