Was fighting with this last night and it appears that my problem was that the Microsoft Mouse driver "intercepts" the middle mouse button and doesn't let it through to X11. (I have an old IntelliMouse with IntelliEye).

I was trying to map middle button to option-click, but in the end have done it with USB Overdrive, and X11s "emulate 3 buttons". Map the middle button to Option-Click and away you go.

Doesn't this work differently?

That is, the 3-button emulator works so that

option-click = middle button
command-click = right button

which, if you're using your left hand to hold down the keys, makes sense, as option is left of command.

I find this a hassle (since I'm now trying to use OOo), so I've used Microsoft Mouse PrefPane (OK, not really a prefpane) to map the right button to command-click. This way, I get the expected contextual menu when I right-click.

I'm hoping that apple adds the ability to use all the buttons on my mouse in the next go-round of X11.
