Actually, this does affect the background program state. I have to do this occasionally when Mail hangs or is having trouble with displaying content. You can also try it with the iPod app or Pandora. Removing it from the "recent item" bar stops audio from playing. You can also check this against running processes if you have iStat installed.

However, if a program has a traditional saved state (from pre-iOS4 days), it will not be affected. But a fast-app switching saved state might not necessarily be preserved.
