OS X, from at least 10.1, but probably earlier, has built-in support for two mouse buttons and a scroll wheel. The scroll wheel support is based on hover focus; that is, the scroll wheel will scroll whatever window/frame/pane/whatever that the cursor is over. I have not yet found a Cocoa or Carbon program in which this does not work (but it doesn't appear to work in Classic unless you have Mouseworks or USB Overdrive or such installed).

Terminal is a rare exception; you have to bring a window to the front to scroll it with a scroll wheel. Sigh.
