I do this all the time. Here's what I do:

In my ~/.ssh/config file, I have:
host nest
hostname eagles.nest.com
user eagle
Cipher blowfish
Compression yes
LocalForward 5022 boston:22

host boston
hostname localhost
port 5022
Host "eagles.nest.com" is the publicly accessible system. Host "boston" is the unreachable system.

So, in one window, I issue the command "ssh eagles.nest.com," and in another window I issue the command "ssh boston."

Et voila.
