Interestingly, the startup tone that plays during a restart still plays through the internal speakers even if there are headphones plugged in.

It didn't before. I think Apple rethought the startup chime:

  • more and more Macs are plugged into (potentially really loud) external speakers, TVs, etc
  • always having it come from the internal speaker simplifies troubleshooting - one of the primary reasons for the existence of the startup tone

I use a great, simple preference pane to keep the startup sound from being too loud:
