Album art in iTunes 7 is full of surprises. I found that if you create a video from a DVD (with a tool such as Handbrake) then add it to your iTunes library, you may have an artwork problem if the DVD has the same name as an album.
I ripped U2's Rattle and Hum DVD today to iPod format, then added it to my iTunes library. It showed up in the Movies library, but, even after I added the DVD cover as artwork, the iTunes Cover Flow display was showing the CD album art (I also have the CD in my library.) I guess iTunes is simply looking at the album name to determine which graphic from its local cache (~/Music -> iTunes -> Album Artwork) to display. Even though the art panel at the lower left of the iTunes window showed the correct art, the Cover Flow graphic was incorrect.
The fix is simple: just change the album's name. In this case, I changed "Rattle and Hum" to "Rattle and Hum " (note the trailing space). The correct art now displays in the Cover Flow view.
