Create an iCal calendar of Address Book birthdays Apps
I have written a small program that builds an iCal calendar with all birthdays from the Address Book. See this page for more information on how to use the tool and a download link.

[robg adds: This worked as described; I installed the program in my user's bin folder, and told it to create a new test calendar. I then switched to iCal and used File: Import to bring the new calendar in. I chose to bring it in as a new calendar, though I could have added its events to an existing calendar.]

There's an AppleScript called ABtoiCal that already does this. I use it and it works well. It handles non-birthday dates, too.

You could also use the excellent ical birthday shifter.

Although, it doesn't check to see if the birthdays area already in the calendar, so you can get duplicates.

At least it does keep track of the birthday entries it adds itself, so you don't get duplicates across repeated runs. If you're worried about getting repeats the first time, just create a new calendar for the program to populate.
