Use Apple's iDVD Companion to align buttons and more Apps
I couldn't figure out any possible way to align buttons in iDVD 4 and then I ran across iDVD Companion on Apple's website. It's an AppleScript app which you launch and use along with iDVD. It appears to have been written for iDVD 2.1. In addition to aligning buttons, iDVD Companion allows you to "nudge" buttons and text in very small increments, and exactly position Title text on the screen. I have not tested all the functionality with iDVD 4, but the button alignment works perfectly.

The instructions are very easy, so just download it and get started!

Alignment Caveat

One thing to be careful of is that for at least some fonts (i.e. the ones I've tried :-) the final rendered text is slightly *different* in size than what you see onscreen within iDVD. (At least, it's this way in iDVD 3 - I don't have 4 yet.)

I ran into this after using the Companion to carefully line up all my titles, then when I burned the DVD I found things shifted. ARGH!

The text alignment setting mitigates this somewhat - if you're using the Companion to line up right edges for example, make sure the text is set to be right-aligned and you'll be ok. (It sounds obvious, but it's easy to overlook.) Just remember you cannot depend on the final rendered title being the exact same size (mainly lengthwise) as what you see in iDVD.
